The Program Support Specialist - SDOH develops, implements, and coordinates programming and strategies around addressing social determinants of health for patients at Jordan Valley.
The Program Support Specialist - SDOH provides technical support and training for key patient engagement roles directly addressing social determinants of health.
This individual also compiles and maintains a set of measures to gauge the prevalence of SDOH-related needs and ensures the list of available community resources is maintained and complete.
In short, the Program Support Specialist - SDOH serves as the champion and subject matter expert in the area of social determinants of health.
Duties:
Support development and implementation of strategy and plans for addressing social determinants of health, emphasizing consistency across the organization.
Develop, maintain, and report on SDOH-related measures, including monitoring the prevalence of SDOH-related needs, compiling and analyzing PRAPARE data, tracking referrals for outside resources, and reporting on patient trends.
Provide technical support and training as a subject matter expert in the area of SDOH, ensuring key patient-facing roles are adequately equipped/trained to address social determinants of health.
Assist, as needed, in the preparation of grant-based funding proposals and associated reporting.
Coordinate partnership and collaboration with external organizations to ensure Jordan Valley has an adequate network of referral sources for patients with SDOH-related needs, and ensures Jordan Valley is actively participating in community efforts to address these factors.
Maintain a database/toolkit of available resources, ensuring patient-facing staff are equipped to assist patients presenting with SDOH-related needs, either directly or through referral to an outside organization.
Participate in quality improvement activities associated with social determinants of health.
Provides programmatic supervision of frontline staff working in the area of SDOH, including providing feedback to line managers on performance of individuals in these roles.
Demonstrate commitment to the broader Jordan Valley team to facilitate the Clinic's ability to meet its goals and objectives.

Qualifications:
Bachelor's Degree in Social Work, Public Health, Healthcare Administration or a related field.
3
years of experience in roles related to addressing social determinants of health.
Experience working for or with community-based non-profit/service organizations.
COVID Vaccine required for all new employees.
Preferred
Qualifications:
Master's degree in public health.
